As part of the 1st phase range, we can now provide an additional service that is beneficial to both clinicians and clients. This new system allows the clinician to refer patients to the foothealthcare.com website to purchase prescribed orthotic products. We aim to work with industry professionals to ensure that patients get the right products for their treatment in an easy, convenient and safe environment.

How to apply;

Clinician / Practitioner

• Please complete Part 1 of the form and photocopy as needed. Forms are also available online at foothealthcare.com
• After examining and diagnosing your client please select the product that your client needs from the 1st Phase brochure.
• Refer to the accompanying 1st Phase price list for the code and price of your chosen product and enter the details in Part 2 along with the product name and quantity needed for the patient. (All orthoses are sold in pairs).
• Please ensure ALL details are filled in correctly to ensure that the order can be processed quickly and efficiently.

USING MAIL ORDER OR FAX TO MAKE A PURCHASE

• Please complete Part 3 of the form including full credit or debit card details AND at least one form of contact other than your address in case we need to contact you regarding your order. If paying by cheque please make out to A. Algeo Ltd.
• Either fax or post the completed form to A. Algeo Ltd, Sheridan House, Bridge Industrial Estate, Speke Hall Road, Liverpool, UK, L24 9HB

USING SECURE ONLINE SHOP TO PURCHASE

• Visit foothealthcare.com and click "REFERRED PATIENTS" where you will find a secure page. Please enter the following details to access the online shop for referred patients: Username : foothealth, Password : referral1973
• On accessing the shop you will have to enter your clinicians details before proceeding to purchase, so please ensure you have your referral form ready when shopping online.
• Then simply find the product you have been prescribed, enter the quantity of pairs you require and click 'BUY'. Follow the online instructions from here to complete the purchase.

Important please note:

Please use this form for referral use only. If you wish to place an order as a practitioner please refer to the back of the 1st Phase brochure. Note that all products sold to patients referred to us through practitioners will be sold at list price.

All orthoses in the 1st Phase range are preforms that can be adapted by clinicians if needed. Algeos do NOT have the facility to modify or adapt any orthoses therefore please only refer patients who require an off-the-shelf orthotic device that requires no modification and can simply be placed in the shoe by the patient themselves.

The Referral System only applies to those that live in the UK or Ireland. The 1st Phase Referral System is simply an additional service we provide for our customers who wish to refer their patients to purchase the correct orthoses that require no modification.

The 1st Phase Referral System is a convenient service for our customers and should not replace full examination, diagnosis and treatment from the clinician. The purpose of a 'Functional Orthotic Device' is to alter the biomechanics of the limb to which it is applied. We believe that 'Functional Orthotic Devices' should be supplied by a trained clinician. We have information on all our products and are happy to answer any request for technical support or general assistance. However all our products are supplied on the basis that the customer understands the concepts of orthotic treatment.

It is the policy of A. Algeo Ltd not to supply functional components or orthotics directly to the general public unless referred by a trained medical professional. All products in the brochure are included in the Referral System excluding the Podo Wedges and Wedge Strips.
